The Hardin County Sheriff’s Office has received numerous reports of persons receiving tax form 1099R from the Ohio Department of Job and Family Services, for unemployment benefits for which they never applied nor received.
This is the likely result of an unknown person making fraudulent applications for unemployment benefits in the name of the person receiving the 1099R.
If you have received this form but never received unemployment benefits, you should contact Job and Family Services at unemployment help.ohio.gov and click on “Report Identity Theft” to complete the form.
