Community Health & Wellness Partners Appoints New Chief Operating Officer

BELLEFONTAINE, Ohio – (June 28, 2021) — Community Health & Wellness Partners (CHWP), an independent, nonprofit health center that provides primary care and other wellness services to the residents of Logan and surrounding counties, has appointed Kathy Smith as its new chief operating officer.

As COO, Smith will contribute to the successful achievement of the organization’s mission and strategic initiatives by providing leadership, oversight, evaluation, and direction for the general administrative functions, clinic operations and change management initiatives. She also will oversee overall operations and integrated ancillary services through a TeamCare model of delivery to ensure patients receive the optimum quality of care, utilizing evidence-based best practices.

Previously, Smith worked at Summit Behavioral Healthcare as the nurse executive in Cincinnati, Ohio. Prior to Summit Behavioral Healthcare, Smith held various roles at Jewish Hospital in Cincinnati, most recently serving as the chief nursing officer, vice president of patient services. She received her Executive Nursing Practice certification (CENP) from the American Organization for Nursing Leadership Credentialing Center and holds a master of science in nursing administration and bachelor of science in nursing from the University of Cincinnati. She is past president of the Greater Cincinnati Nurse Executives and a past board member of the Ohio Organization of Nurse Executives.

Smith and her husband Tim Smith, who is recently retired, enjoy lake life at Indian Lake and have visited the area with their family for nearly 20 years.
