USV Reminds Parents About Student Vaccinations

Upper Scioto Valley parents, who have students in Kindergarten, 7th, or 12th grade are being asked to make sure that they are up to date on immunizations. 

 

If you choose not to vaccinate your child there is a waiver that needs to be signed every school year. 

 

The form can be picked up at the school, mailed to you, or sent home with your child. 

 

To request the waiver form, call the school. 

 

If your child has received their immunizations, you should request the records and send them to the school. 

 

Your physician’s office may fax the records to 419-757-0134.
