The Mental Health Drug & Alcohol Services Board of Logan and Champaign Counties is conducting a survey as they work to gain more insights about how the March 14 Indian Lake tornado affected your mental health.

They hope you can help them learn how they can serve the community in the most meaningful way, supporting long-term recovery efforts that are in place.

If you live or work in the region, were a first responder, social worker, or volunteer who helped with response, or have family or friends impacted, take a few minutes to complete this survey.

Your response is anonymous, and your email address will not be collected as a part of your response.
