Lima Woman Suffers Life-Threatening Injuries in Allen County Collision

Amanda Twp – On July 14, 2026, at approximately 5:42 P.M., the Lima Post of the Ohio State Highway Patrol responded to a report of a two-vehicle crash on Deep Cut Rd and South Defiance Trail, Amanda Township, Allen County.

Grace Cook, age 24, from Lima, Ohio was westbound on Deep Cut Rd driving a 2015 Jeep Compass. Caleb Hembree, age 29, from Lima, Ohio was northbound on South Defiance Trail driving a 2012 Jeep Patriot. Mr. Hembree failed to stop at a posted sign and struck the vehicle driven by Ms. Cook. Ms. Cook suffered life threatening injuries and was transported from the scene by Spencerville EMS to Mercy Health St. Rita’s in Lima. Mr. Hembree was not injured in the crash.

The Jeep Compass sustained severe damage and was towed from the scene by Tops Towing. The Jeep Patriot sustained severe damage and was towed from the scene by Tom’s Towing

The Ohio State Highway Patrol was assisted at the scene by the Shawnee Township Police Department, the Spencerville Fire Department, Spencerville EMS, Tops Towing, and Tom’s Towing.

The crash remains under investigation.  The Ohio State Highway Patrol reminds motorists not to drive distracted and to stop at stop signs.
