Consent Forms Sent Home with USV K-6 Students
Health Partners of Western Ohio Dental and Vision Outreach consent forms were sent home Wednesday with all Kindergarten through 6th grade students at Upper Scioto Valley.
The program is also open to 7-12th grade students as well.
If you would like a form sent home with your 7-12th grade student, call the office at 419-757-3231 x 3.
The forms need to be signed and returned to the school.
Health Partners will bill Medicaid and private insurance.
If your child has no health coverage, there will be no charge.
